Employee Health Plan – Tobacco Treatment 

The Tobacco Cessation Program is administered through the Employee Walk-in Clinic.   However, all tobacco cessation appointments must be scheduled in advance.   Call 216.636.2424 to schedule an appointment.   Telephonic appointments are available.  Any testing must be prior approved by the Employee Health Plan. 

• At your first appointment, complete the EHP Smoking Cessation Application, using this date as your start date.  If the application is not completed, payment of any claims will be denied.  You can obtain an application from the clinician you are seeing at the Walk-in Clinic and they will forward to the Employee Health Plan.
• Cotinine testing (urine test) will be determined by the tobacco program coach, depending on quit date, and performed for the Healthy Choice Program in August-September (must be performed by EHP approved tobacco treatment provider).  Eligible members will be sent a notification by EHP to schedule their cotinine test.
